Possible COVID-19 Exposure

RE: Possible Coronavirus (COVID-19) Exposure Notification

 

Pursuant to California A.B. 685, we need to inform you that an individual who was in our facility on February 4, 2021 to whom you may have been exposed has been diagnosed with COVID-19.  Please understand that in accordance with federal and state health privacy laws we cannot share any information regarding the identity of the impacted individual or related details.

The health and safety of our employees is our primary concern. In accordance with health guidelines, we are in the process of conducting additional deep cleaning and sanitizing in addition to the current infection control practices and principles outlined in our infection control policies, copies of which are available to you upon request.

Should you exhibit any of the COVID-19 symptoms noted below, please contact your health care provider and notify your supervisor:

  • Fever or chills
  • Cough
  • Shortness of breath or difficulty breathing
  • Fatigue
  • Muscle or body aches
  • Headache
  • New loss of taste or smell
  • Sore throat
  • Congestion or runny nose
  • Nausea or vomiting
  • Diarrhea

Should you find yourself unable to work as a result of being diagnosed with or exposed to COVID-19, please contact your supervisor or HR/Payroll Representative.

Also, please be advised that an employer shall not retaliate against a worker for disclosing a positive COVID-19 test or diagnosis or order to quarantine or isolate.  Workers who believe they have been retaliated against in violation of this section may file a complaint with the California Department of Labor Standards Enforcement.
